Template to Allow Users to Choose Their Prize?

Hello,

I'd like to do a 'choose your own prize' contest – where there are five different prizes, and the users select the one they'd like to win, then fill out an entry form to be entered to win. After they will have the opportunity to share their entry (and the prize they chose) with their social media accounts.

So in the end we'll end up with a segmented list of entrants, based on which prize option they selected, and they'll be able to share some of their personality with their followers.

Is there a pre-built template that can be repurposed to do something like this? I'm not great with the system (very new to SS), but I should be able to figure out some minor tweaks, I'd just like to have a headstart via template.
